Diabetes Case Reporting: For Alaska Native Health Corporations Only

When you have a  new case of diabetes, impaired fasting glucose (IFG), impaired glucose tolerance (IGT), gestational diabetes (GDM), or other elevated abnormal glucose in your site/area, please fill out the Diabetes Case reporting form  accurately and completely.   Please review the instructions on how to fill out the form if you are unfamiliar with the process. 

Mail it to: Joan Hastie, Alaska Native Medical Center Diabetes Program (ANC-DIA), 4315 Diplomacy Drive, Anchroage, AK 99508.

Fax it to the Alaska Area Diabetes Program at 907-729-1129 to the attention of the Registry Team. 

When we receive the form, it is checked for accuracy and then entered into the statewide diabetes registry.  You may get questions from the Diabetes Registry Team if any of the information is missing or needs clarification.  This process helps both you and us to keep track of the number of cases and new cases every year, and helps in clinical follow-up, case management, funding and advocacy.
